Vascular Lasers for Rosacea and Facial Redness: A Complete Treatment Guide
Vascular lasers treat facial redness by delivering light that is absorbed preferentially by haemoglobin, heating the blood vessel until it collapses while leaving the surrounding skin largely intact. Pulsed dye lasers, potassium titanyl phosphate lasers, long-pulsed neodymium-doped yttrium aluminium garnet lasers, and intense pulsed light devices are all used for this purpose, and Korean clinics commonly offer several of them under trade names rather than technical ones. They are not interchangeable. The right device depends on whether the redness is diffuse or vessel-specific, how deep the vessels sit, and the patient's skin type. What is actually causing the redness
Facial redness has several distinct causes that look similar in a mirror and respond very differently to light. Diffuse erythema is a generalised flush across the cheeks, nose, and central face caused by many small dilated vessels. Telangiectasia describes individually visible vessels, often on the nasal alae or cheeks, fine and linear or branching. Flushing is episodic and neurovascular rather than structural, and it is the component that lasers address least well.
Rosacea is a chronic inflammatory condition with subtypes that combine these features with papules, pustules, or, in some patients, thickening of nasal skin. Post-inflammatory erythema left behind by acne is a different entity again, as is redness from a compromised skin barrier or from overuse of topical steroids. A treatment plan that does not name which of these it is targeting is unlikely to be aimed correctly.
The mechanism: selective photothermolysis
The governing principle is selective photothermolysis. A target absorbs a chosen wavelength more strongly than the surrounding tissue, and if the energy is delivered in a pulse shorter than the time the target needs to cool, heat is confined to the target. For vascular work the target chromophore is oxyhaemoglobin, which has absorption peaks in the region of roughly 418, 542, and 577 nanometres, with a broader shoulder extending into the near infrared.
Two variables then decide the outcome. Wavelength determines both absorption and depth of penetration, with longer wavelengths reaching deeper but absorbed less strongly. Pulse duration must be matched to vessel calibre, since a fine telangiectasia cools far faster than a thick one and requires a shorter pulse to be damaged selectively. Fluence, the energy delivered per unit area, sets how much heat arrives. Getting these wrong produces either no effect or injury to the surrounding skin.
Pulsed dye laser: the reference device for diffuse redness
Pulsed dye lasers emit at approximately 585 to 595 nanometres, close to an oxyhaemoglobin absorption peak, and are widely described in dermatological literature as a first-line option for diffuse facial erythema and for the erythematotelangiectatic presentation of rosacea. The wavelength penetrates to the depth at which most superficial facial vessels sit.
Settings determine the experience. Purpuric settings, using short pulses and higher energies, rupture vessels and leave bruising that typically resolves over one to two weeks, and are generally reported to produce more clearance per session. Sub-purpuric settings use longer pulses and lower energies with little or no bruising, at the cost of needing more sessions. Many clinics now default to the second approach for patients who cannot accept downtime. Multiple sessions, often spaced several weeks apart, are usual either way.
KTP and Nd:YAG: targeting individual vessels
Potassium titanyl phosphate lasers emit at 532 nanometres, very near a haemoglobin absorption peak, which makes them efficient for fine, superficial, individually visible vessels. The same strong absorption is also taken up by melanin, so the risk of pigmentary change rises in darker skin and the device is generally used cautiously in Fitzpatrick types IV and above, which includes a large proportion of Korean and other East Asian patients.
Long-pulsed Nd:YAG lasers emit at 1064 nanometres. Haemoglobin absorbs this wavelength far less efficiently, which sounds like a disadvantage but is the reason it can reach deeper, larger vessels that shorter wavelengths cannot, and why it is comparatively safer in pigmented skin. It is commonly selected for thicker blue or deep vessels. The trade-off is that higher energies are required, making the treatment more uncomfortable and less forgiving of technique errors.
Intense pulsed light
Intense pulsed light is not a laser. It emits a broad spectrum filtered to a chosen range, which allows it to address both vascular and pigmented targets in the same pass and makes it efficient for generalised background redness with sun damage. Because the output is broad rather than monochromatic, it is generally described as less precise for individual vessels than a dedicated vascular laser, and operator settings vary widely between devices.
What results are realistic
Published discussions of vascular laser treatment for rosacea typically describe meaningful reduction in erythema and clearance of visible vessels over a course of sessions, rather than a single definitive cure. Most protocols involve three or more sessions spaced several weeks apart, with the number depending on severity and device.
Two expectations need setting clearly. First, lasers treat the visible vessels present today; they do not alter the underlying tendency, so new vessels can develop and maintenance sessions are commonly needed over years. Second, flushing responds least predictably, because the episodic neurovascular component is not a structural target. Patients whose principal complaint is heat, stinging, and sudden flushing rather than fixed redness should have that discussed honestly before committing to a package.
Risks, and the specific caution for Asian skin
The most relevant complication in East Asian skin is post-inflammatory hyperpigmentation, where inflammation from treatment triggers melanocytes to produce pigment that can persist for months. Risk rises with higher melanin content, with aggressive settings, and with sun exposure after treatment. Assessment of Fitzpatrick skin type and test spotting are standard precautions, and diligent photoprotection afterwards is not optional.
Other reported risks include blistering, crusting, transient hypopigmentation, and scarring, all uncommon with appropriate settings but not zero. Purpura is an expected effect at certain settings rather than a complication. Patients with a recent tan, active infection, or recent isotretinoin use are usually asked to defer, and photosensitising medications should be disclosed.

Because rosacea is chronic, laser treatment usually sits inside a broader plan.
