Recognised risks include nerve-related sensory changes affecting the lower lip and chin, asymmetry, and bleeding. It is not reversible.

A frequently overlooked consequence is that removing bony support in an older or thin-skinned patient can accelerate the appearance of soft-tissue laxity along the jawline. This is one reason age and skin quality, not just bone shape, are part of candidacy assessment.

Option 2: Masseter Reduction for Muscular Width

When width is muscular, botulinum toxin injection into the masseter is the usual first-line approach. Reported onset is gradual over 2 to 6 weeks as the muscle reduces in volume with disuse, and effects commonly last 4 to 6 months before repeat treatment is required.

The diagnostic test is simple and can be performed before any consultation. Clench the jaw firmly and palpate the angle. If a distinct muscular bulge appears and disappears on relaxation, a muscular component is present. If the width is unchanged between clenched and relaxed states, the contributor is more likely bone.

The trade-offs are the mirror image of bone surgery. It is non-surgical, has essentially no downtime, and is fully reversible over months, but it is temporary, produces a moderate rather than dramatic change, and has no effect on bone. Long-term repeated treatment carries a recognised possibility of some chewing-efficiency change, which is worth discussing if treatment is planned over many years.

Option 3: Buccal Fat Removal for Mid-Cheek Fullness

Buccal fat removal targets a discrete fat pad in the deep cheek, usually via a small intraoral incision. It narrows the mid-to-lower cheek and can enhance the hollow beneath the cheekbone. Downtime is comparatively short, with reported swelling largely settling within 2 to 3 weeks and final contour appearing over 3 to 6 months.

Its main limitation is anatomical location. The buccal fat pad sits higher and more medially than the mandibular angle, so it does not narrow the jaw itself. A patient whose primary concern is the angle will generally not be satisfied by buccal fat removal alone.

The most important caution is age-related. The procedure is not reversible, and facial fat volume typically declines with age. Aggressive removal in a patient in their twenties may contribute to a gaunt mid-face appearance two decades later. Conservative removal is the prevailing recommendation for this reason.

Option 4: Non-Surgical Tightening for Laxity-Driven Width

When the jawline is blurred by skin laxity rather than by bone, muscle, or deep fat, energy-based tightening such as high-intensity focused ultrasound or radiofrequency microneedling, or thread lifting, addresses the relevant layer. These options reposition or tighten soft tissue rather than reducing volume.

They are generally most appropriate for patients in their thirties to fifties with mild to moderate laxity and reasonable skin quality. Results are typically gradual, partial, and temporary, with repeat treatment commonly required at intervals of 12 to 24 months depending on modality.

What Most Guides Leave Out: The Diagnostic Sequence That Prevents Wrong Procedures

Competing articles usually compare procedures on cost and downtime. The more useful comparison is diagnostic, because selecting the wrong layer produces a poor result regardless of surgical skill. A four-step sequence covers most cases.
